Description

On macOS, trying to start any local agent from Agent Manager fails with the error:

Failed to start agent session: Session creation timed out - CLI did not respond

I have verified:

• Kilo CLI is installed and runs successfully from both VSCode integrated terminal and a regular terminal session.
• Issue persists regardless of workspace or repository.
• VSCode and Kilo are both up to date.

There may be an issue with how the extension locates or communicates with Kilo CLI on macOS, but my PATH and CLI install are both accessible. This could be an OS integration or process communication issue.

Reproduction steps

1. Run Kilo CLI directly to confirm it is working.
2. Open VSCode, open Agent Manager.
3. Attempt to start any local agent.\n4. Observe error: 'Failed to start agent session: Session creation timed out - CLI did not respond'.
