Requests Dashboard: We should try to limit max width so the table never scrolls?

• approve / deny shouldn't be at the end or you shouldn't have to scroll so much,
• should we have icons for view/approve/deny?
  • "-->" for view, "√" check icon for approve, and "X" for reject" (still blue/green/red)
• what columns should collapse first in smaller views?
