Remove all the refresh logic for measures, libraries, plandefinitions, questionnaires, etc. from the CQF Tooling, all that is done in the publisher now and we should delete it all from CQF Tooling, including anything related to "create Library from CQL", "create Measure from CQL".
Add a packageArtifact operation that makes use of clinical-reasoning module's CRMI package implementation, and works off of NPM Packages (so the FHIR local cache for a ci-build). For now, just run off of the vocabulary directory in the local IG, We'll consider adding terminology endpoints to the process as a future enhancmenet.
