Pagination not working correctly for listing incidents

Issue Description

A user has reported that the pagination for listing incidents is not working as expected in the incident.io MCP server.

Problems Reported:

• The maximum page size appears to be limited to 5 items
• The cursor functionality is not working correctly, preventing navigation to page 2 and beyond

Expected Behavior:

• Pagination should allow navigation through all pages of incidents
• The cursor should correctly move through the result set
• Page size should be configurable or at least allow for larger pages than 5 items

Steps to Reproduce:

1. Use the incident.io MCP server with Claude Code
2. Attempt to list incidents with pagination
3. Try to navigate beyond the first page using the cursor

Additional Context:

This issue was reported by a user testing the incident.io MCP server integration with Claude Code.
