2 issues:

  1. make sure USDT@ETH/USDT@ETH kind of tickers are properly handled (not fetched, like USDT/USDT)
  2. handle coinrabbit's market orders taking 30s to 2min to settle (order_utils fix, requires [CoinRabbit] fixes ccxt#43 to work with coinrabbit)
