CMS-0057 Add appointment-book hook documentation

Expands CRD CDS Hooks API documentation to cover additional workflow hooks beyond order-sign.

• New sections: order-select, order-dispatch, and appointment-book
• For each hook, adds: endpoint (POST [base]/cds-services/...-crd), context tables, request parameters, and example request/response payloads
• Examples include realistic FHIR Bundles (MedicationRequest, ServiceRequest, Appointment, Task) and Cards with suggestions/links
• Notes that actual card content depends on CDS_DECISION_SERVICE_URL
