feat(skills): add agent-qa testing workflow pack

• Adds one source-backed skills entry for agent-qa's authoring, result-triage, and debug-fix workflow pack.
• Documents live retrieval sources, prerequisites, supported agent clients, runtime providers, safety/privacy constraints, and the project's FSL-1.1-ALv2 with Apache-2.0 future license.
